To withdraw your child from Stevenson Middle School, please complete the Request to Release Form. There are 2 options to complete the form.
Come to the Registrar’s Office to complete it.
Complete the Request for Release Form online. You will need to attach a copy of your picture ID.
A copy of the completed form will be emailed to the Registrar after you verify your email.
All textbooks, library books, and chromebooks must be returned and obligations cleared before a release packet will be provided.
Please complete the form at least 72 hours before the student’s last day of attendance.
If you do not complete the form beforehand, please be aware that there may be a delay in sending records to your child’s next school.
