Student Information Update Form

To update your child's information at Stevenson Middle School, please complete the Student Information Update Form (SIS-11). There are 2 options to complete the form.

1. Come to the Registrar’s Office to complete it.

2. Complete the Student Information Update Form online. If you are updating your residential address or other legal documents, you will need to attach a copy of your proof of residence or legal document. 

   1. • A copy of the completed form will be emailed to the Registrar after you verify your email.
