1) Release packet from prior HIDOE school

2) SIS-10W - Enrollment Form  (submit electronically using Adobe Sign)

3) Verification of Residency Statement (attach proof of residence)

4) Addendum to Enrollment (if needed, provide legal documents)

5) MV1 Form
