Student Enrollment Form (SIS-10W)

To update your child's information at Stevenson Middle School, please complete the Student Enrollment Form (SIS-10W). There are 2 options to complete the form.

1. Come to the Registrar’s Office to complete it.

2. Complete the Student Enrollment Form online. You will need to attach a copy a guardian's ID and a proof of residency--a utility bill, lease agreement, or other official documents showing your current address in the guardian's name.

   1. • A copy of the completed form will be emailed to the Registrar after you verify your email.
